The First Blade is the only thing that can kill higher level demons such as the Hellknights. As it is the demon-equivalent of an arch-angel blade, it will probably be able to kill them too. Also, Death said that one day, he may even reap God.

Because, as Crowley informs Dean, the human body was never meant to contain the First Blade's power. After Metatron kills him, Dean is resurrected by the power of the Mark and the First Blade, which twisted his soul until it was demonic. As a demon Dean showed no care for anyone or thing -- not even Sam or the Impala.
