The First Blade is the only thing that can kill higher level demons such as the Hellknights. As it is the demon-equivalent of an arch-angel blade, it will probably be able to kill them too. Also, Death said that one day, he may even reap God.

Because, as Crowley informs Dean, the human body was never meant to contain the First Blade's power. After Metatron kills him, Dean is resurrected by the power of the Mark and the First Blade, which twisted his soul until it was demonic. As a demon Dean showed no care for anyone or thing -- not even Sam or the Impala.
It's not because of Sam's purified blood, because in Season 8 Episode 2, when Crowley gets out of Mrs Tran. However, Sam only injected his purified blood to Crowley at the end of Season 8, so his smoke is always red.
The road so far has come to a devastating end: Supernatural wrapped up its 15-season run on Thursday with a series finale that found Dean dying during a hunt and saying an early goodbye to his brother Sammy, who went on to live a long life.
